    The VNA is seeking a per diem Community Wellness Nurse to promote community health and wellness by conducting blood pressure (BP) and blood sugar (BS) screenings, health fairs or immunization clinics at locations in Indian River and Brevard counties.


    1. Blood pressure and blood sugar screenings:
    • Maintain an accurate calendar of assigned BP/BS clinics.
    • Procure, store and maintain clinic supplies in assigned VNA bag in a safe and appropriate manner.
    • Arrive at the assigned clinic no more than 15 minutes ahead of start time.
    • Perform blood pressure and blood sugar check according to the department blood sugar and blood pressure procedure manual.
    • Distribute up-to-date teaching literature and discuss the importance of maintaining healthy blood sugar and blood pressure levels. Discuss disease prevention, control and the importance of testing/monitoring, as appropriate.
    • Complete a medical referral form if deemed necessary.
    • Clean up work area at the completion of the clinic.
    • Return clinic sign-in sheets and any referral forms to the Community Wellness Manager.

    2. Immunization clinic role:

    A) Immunization Clinic Site Manager (RN only)

    • Call the Community Wellness Manager at least one week in advance of assigned clinic to schedule an appointment to pick up serum and supplies. Multiple clinics can be picked up at once. At the same time, arrange for return of inventory within 48hrs of clinic.
    • Call the assigned clinic team to ensure that they have directions and are planning to attend.
    • Call the clinic contact person before the clinic to confirm and get any last minute instructions.
    • Arrive at the clinic site 30 minutes prior to start time. (Fifteen minutes may be adequate for small private clinics)
    • After arriving at the clinic, report to the contact person to determine location and clinic flow so it will not disrupt the store or business site.
    • Set up posters/signs to advertise the clinic. (Do not place posters at the road during private clinics unless inside a gated community)
    • Supervise and assist in setting up the work area. Arrange tables, tape waste bag on the edge of nursing station, set out chairs for clients to rest after vaccination. Arrange medical supplies and forms.
    • Act as a resource for clerical personnel.
    • Perform bullets 3-6 of Immunization Clinic Nurse (2b.), as needed.
    • Count money and consent forms to verify the clerk’s information on the Clinic Worksheet.
    • Verify that the vaccine log is complete with totals and signatures.

    B) Immunization Clinic Nurse (RN or LPN)

      • Arrive at the clinic site prior to the scheduled start time (30 minutes prior to large clinic or 15 minutes prior to small).
      • Report to site manager and assist in setting up the work area - arrange medical supplies.
      • Determine that client understands the consent form and answer questions related to vaccine or administration. Check for accurate completion of the consent form by the flu clerk.
      • Administer vaccine adhering to universal precautions.
      • Document dose and injection site and sign/date the consent form.
      • Clean up work area at the completion of the clinic.

    3. Report accidents or unusual incidents immediately to the Community Wellness Manager.

    4. Attend mandatory staff meetings and in-service programs. Fulfill mandatory e-learning courses by the due date.

    5. Perform all nursing duties in accordance with the Florida Nurse Practice Act and comply with VNA and department policies and procedures.

    7. Represent the VNA at Health Fairs; distribute VNA information pamphlets and answer questions about the VNA.


    • Active RN or LPN license in the State of Florida
    • Current Basic Life Support for Health Care Provider Certification/CPR
    • Minimum one year work experience in hospice, home health, geriatrics, or medical/surgical nursing

    Physical Requirements:

    To perform the essential duties, the individual must be able to:

    Regularly: sit, stand, walk, balance, talk, hear, feel and touch; reach and grasp with hands/arms; use hand-eye coordination, use vision for close, peripheral, depth and ability to adjust focus

    Occasionally: stoop, bend, kneel, crouch, crawl and climb; carry/lift up to 25 pounds

    Work Environment: Time is spent at various community sites; moderate noise level. Local travel is necessary within Indian River and/or Brevard counties.
